Birth Certificate
A birth certificate is a confidential document, and only the individual or the parent guardian or someone with a notarized permission statement
can apply for the birth certificate. To obtain a birth certificate you must have the following information:
A valid I.D. Birth given name and Date of Birth of the applicant
Mother’s maiden name of applicant.
Father’s full name of applicant.
If requesting the Birth Certificate for someone else we will need to know what your relationship is to the
applicant.
Fees for Certified Birth Certificates: $25.00 for the first and $5.00 for each additional certified copy.
Cash, Cashier’s Check or Money Order (check made out to The Probate Court).
To obtain your birth certificate by telephone, mail a copy of your Driver’s license or picture I.D.,
please include your birthgiven name and date of birth, mother’s maiden name, and your
father’s full name to The Probate Court.
